I confirm that I have read and fully understand this tattoo client consent and liability form. I agree to comply with all instructions given by the tattoo artist regarding the care of my tattoo, including aftercare guidance. I understand that the tattooing process involves piercing the skin with a needle and depositing ink into the skin, which may cause pain, discomfort, swelling, and bleeding. I understand that there is a possibility of an allergic reaction to tattoo ink, and I confirm that I have informed the tattoo artist of any known allergies or medical conditions. I understand that the tattoo artist has explained the risks of infection and has provided instructions for proper aftercare. I understand that tattoos may not be removable completely and that any attempt at removal may cause scarring, skin discolouration, or other complications. I understand that the final result may vary from the initial design due to factors such as skin type, healing, and individual response. I hereby release the tattoo artist and TattLabStudios from any and all liability including injury, infection, allergic reaction, or damage to property arising from the tattoo process. I understand that the tattoo artist is not responsible for any medical expenses incurred as a result of complications. I confirm that I am 18 years of age or older and have provided valid photographic identification. I confirm that all information I have provided is accurate to the best of my knowledge.
